Your First Visit

When you visit our office for the first time, we’ll perform a full dental examination before your consultation with Dr. Sohn. Then, Dr. Sohn will go over your medical history, discuss your smile goals, and review your treatment options. Your first appointment gives us a chance to get to know you better and understand your dental needs.

New Patient Forms

• Fill out our new patient forms online and they will be submitted electronically
• Create a list of any current medications you’re taking
• Have your previous dentist send us your past X-rays (if any)
• Make sure you have your up-to-date dental insurance card
